Program Overview

 

The link worker will work with the Older Adult to cocreate a care plan to ensure that supports can be in place in the areas of; food security, case management, home and yard maintenance, financial assistance, service navigation and social and recreational programs.

 

Referral Process

A regulated health profession provides a social prescription to an Older Adult (55+) to receive aging in community supports.
